    Forget Me Not cards were designed to help people living with dementia, with learning difficulties or physical disabilities as well as those who are too poorly or frail to communicate their needs and preferences when they are in hospital.

    MATRIX Neurological is a registered charity based in Middlesbrough that works Tees Valley wide. Lived experience led we support children and young people living with the effects of an acquired brain injury following an accident or illness. We also provide peer-led practical and emotional support to their families.

    In February 2024 North Tees & Hartlepool and South Tees Hospitals NHS Foundation Trusts formally came together to work in partnership. This is an exciting opportunity for the two trusts to work together to strengthen their collaborative offer of patient care.
