Why did you decide to become a Community Champion? 

I decided to become a Healthwatch Community Champion so that I can act as a voice for the people I support. I hope to work with other Community Champions to address and remove barriers encountered to make South Tees a healthier and happier place to live.

How does your role help people in South Tees? 

I am a Social Prescribing Support Practitioner in the East Cleveland Primary Care Network (PCN). I work with people who are referred to our service by GPs.

By taking a personalised approach and linking in with other community services, I support people referred to me to address the root socio-economic causes which impact on their health and well-being such as debt, housing issues, social isolation and loneliness.
